Parents enrolling students in Mount Washington who have never been to a Baltimore City Public School will need to bring the following for their child:

• Child’s Social Security card
• Child’s birth certificate
• Child’s immunization record (shot record)
• 2 Proofs of Residence (BGE bill, landline telephone bill, mortgage statement, government mail)
• Parent/guardian photo ID

In addition, parents enrolling students in Mount Washington from other schools will need to bring the Maryland Transfer form completed by the sending school.
